Anthem Blue Cross and Blue Shield Foundation Funds Western Reserve Area Agency on Aging $50,000.00 to Raise Awareness about Diabetes Risks


The Centers for Disease Control and Prevention report that more than 29 million Americans have diabetes. Alarmingly, 1 in 4 who has diabetes does not know.  Western Reserve Area Agency on Aging (WRAAA) provides choices for people to live independently in the place they want to call home. 

WRAAA provides community engagement activities like diabetes self-management programs to address the health and wellness of older adults. Thanks to the program sponsorAnthem Blue Cross and Blue Shield Foundation, WRAAA will implement the program, Diabetes Awareness-How Sweet It Is! Through this twelve-month program, WRAAA will: Increase awareness about diabetes risk factors and preventing complications; Increase capacity to implement lifestyle changes; and Promote a community and evidence based approach.

WRAAA is seeking community based partners in Cuyahoga, Geauga, Lake, Lorain, and Medina counties that are interested in reducing chronic disease like diabetes. These types of efforts have been proven to significantly lower hospital admissions/readmissions.
