PASSPORT

WHAT IS
PASSPORT? PASSPORT is a Medicaid waiver program that
provides information to anyone who has questions about long-term care services.
PASSPORT offers comprehensive in-person assessments by registered nurses and
licensed social workers to help older persons, their family members and
caregivers determine what services they need and want.

| PASSPORT is the cornerstone of the Western Reserve Area Agency on Aging's
community-based long term care initiative. This initiative is designed to
provide long-term care options at a lower cost than institutional care. In our
five-county area, including Cuyahoga, Geauga, Lake, Lorain, and Medina counties,
PASSPORT supports more than 3,000 seniors to remain where they want to be--in
their own homes with the independence and dignity they deserve. Our belief is
that no older person should be forced to accept nursing home placement because
of the lack of available homecare services.

It is the mission of the Western Reserve Area Agency on Aging's PASSPORT
program to support and enhance a comprehensive and coordinated system of
appropriate and sound services to address the unmet needs of older persons.
PASSPORT offers two major service components to address the long-term care needs
of low-income older persons:

in-depth screening and comprehensive in-home geriatric
assessments

case-managed home care services for Medicaid-eligible persons 60
years of age or older.

WHO IS ELIGIBLE FOR
PASSPORT?
Anyone of any age can call PASSPORT for
information about long-term care and to help determine what services will
best meet their needs.
A person is eligible for PASSPORT in-home services
if he or she:

is 60 years of age or older

is eligible for Medicaid**

| ð |
lives in a home or apartment
|
| ð |
needs hands-on help with daily activities (i.e. bathing,
dressing, meals) to remain at home.

the person and her/his doctor agree to the care
plan
|
| ð |
health and safety needs can be met by home care
services.
|
| ð |
the entire package of services costs no more than 60% of the cost
of Medicaid nursing home care.
| ** PASSPORT can make
a preliminary decision that a person meets financial eligibility requirements;
however, the county Department of Human Services makes the final
determination.
Persons not eligible for PASSPORT in-home services may be
referred to other community-based services or assisted with nursing home
placement if appropriate. |

CARE CHOICE OHIO
|
WHAT IS CARE CHOICE
OHIO? Care Choice Ohio is a consultation program
designed to promote long-range planning for health and financial care for Ohio's
older population. Far too often, plans for the care of elderly family members
and friends are put off into "the future". Consequently, these difficult and
often stressful decisions are frequently made during a crisis situation and in
great haste. |
| Not long ago in Ohio and nationwide, long-term care for older persons most
often meant only care in a nursing home. Not anymore. When it comes to long-term
care, Ohioans have more choices than ever before. Now, thanks to a statewide
assistance program called Care Choice Ohio, finding out about available services
can be quick and convenient. Care Choice Ohio has been initiated specifically to
help seniors and their family members begin to plan earlier for the potential
long-term care needs of loved ones while there is still sufficient time to take
a calm and informed look at the situation and all of the options and resources
available. |
| Anyone, regardless of age or income, can take advantage of this free service
offered by the Western Reserve Area Agency on Aging through Cuyahoga, Geauga,
Lake, Lorain, and Medina counties. Specially-trained consultants from the
Western Reserve Area Agency on Aging are available to make friendly,
personalized home visits to assess you or your loved one's circumstances and
needs. They will discuss eligibility for programs and will inform you and your
family of the long-term care services and long-range financial planning options
existing in Ohio. |

RESIDENTIAL STATE SUPPLEMENT
"RSS"
|
WHAT IS RESIDENTIAL STATE
SUPPLEMENT? Residential State Supplement (RSS) is a cash
supplement program for low-income adults who are aged, blind or disabled who
need assistance with daily activities but do not need the skilled nursing care
provided in a nursing home. |
| A monthly supplement from the Ohio Department of Human Services, in
combination with the recipient's regular monthly income, is used to pay for
accommodations, supervision and personal care services. The reecipient of RSS
must reside in a certified or licensed facility.

| Each Residential State Supplement recipient has a case manager from the
Western Reserve Area Agency on Aging who maintains regular contact to monitor
the recipient's delivery of services and living arrangement. Recipients also
receive Medicaid benefits. |
WHO
IS ELIGIBLE TO BE A RESIDENTIAL STATE SUPPLEMENT
RECIPIENT?

Persons who are:

aged, blind or disabled

financially eligible for Medicaid

determined to meet a "protective level of care", but who need
less than 24-hour supervision and less than 120 days of skilled nursing care
within a 12-month period

willingness to reside in a certified adult foster home, a
licensed adult family home, group home or rest home, community alternative home,
or mental residential facility
