Alzheimer's Respite Request for Proposals (RFP) SFY 2010
The Western Reserve Area Agency on Aging (WRAAA) at 925 Euclid Ave. Cleveland, Ohio 44115, is soliciting proposals from agencies which provide respite services that assist family caregivers of people with Alzheimer’s Disease or related dementia in Cuyahoga, Geauga, Lake, Lorain, and Medina Counties. WRAAA is soliciting proposals from agencies which provide Personal Care, Homemaker and Adult Day Service, and Alzheimer’s Associations which provide Alzheimer’s Core Services, Education, and Respite Reimbursement. Alzheimer’s Core Services, Education and Respite Reimbursement will be funded exclusively through Alzheimer’s Associations. All documents required to submit a complete proposal are available on this WRAAA beginning Tuesday March 3, 2009. A complete proposal application must be received by WRAAA by 4:00 pm on Friday April 3, 2009 in order to be considered for funding.
